Gym, Pool, and Fitness Class Membership

Community members are invited to join the Sheffield Gym and utilize the facilities by paying a semester membership fee. Membership includes use of the gym, pool, and unlimited fitness classes.
Fields with asterisk are required.

All new members will need to order a membership card. 
Individuals that have lost their memberhship card will need to order a new one.

I understand that I must present my membership card each time upon entering Sheffield Gym or using the Sheffield Pool.
